CONSENT FORMS

Prior to performing a service, you will be asked to fill out a consent form. BLADED by Ashley is not responsible for and assumes no liability in the case of false information given.

AGE RESTRICTIONS

All Microblading or clients wanting to receive Permanent Makeup must be 18 years of age or older, with or without parents’ consent. All other services require a parents’ consent and signature before services will begin.

GUESTS

BLADED by Ashley allows for one guest to be present during the appointment. However, in order to give my full attention to you and your service, kids are not allowed in the treatment room or in the lobby.

CANCELLATION

We ask that you please reschedule or cancel at least 12 hours before the beginning of your appointment or you may be charged a cancellation fee of 50% of services booked. However, we understand that life happens, so please communicate with us if you know that you may be late or are unable to make it to your scheduled appointment and we will do our best to work with you. Of course, schedules do not always allow the availability to be able to provide you with the best service, and at the end of the day, we want you to be happy and we do not want to rush. Clients who arrive more than 15 minutes after the scheduled appointment start time may be asked to reschedule for a later date and may be subject to a cancellation fee.

PAYMENT

We use Square for our POS system. Any and all card information is secured through the Square system and we have no access to your private card information.

BLADED by Ashley accepts payments through cash, debit or credit card, Cash App, BLADED by Ashley gift card or Venmo.
